Louisiana forms is intended to accompany the LexisNexis Louisiana annotated statutes. Louisiana forms should be used along with the Louisiana code of civil procedure, the Uniform rules of Louisiana Courts of Appeal and the Rules for the Louisiana district courts. The purpose of Louisiana forms is to assist Louisiana attorneys in the general practice of law. This work is a general forms book and does not attempt to be an exhaustive source in highly technical practice areas. Rather, these forms are intended as samples of prototype documents and are not a substitute for substantive understanding of individual cases. Applicable forms and captions, therefore, should be tailored and adapted to the specific needs of each practitioner and judicial district court.
